ADOLE7C- AdolesCents reCeiving Continuous Care for Childhood-onset Chronic Conditions

The Adole7C project is a research project that aims to identify risk factors for interruptions in medical follow-up. The Adole7C project is generic in its idea, meaning that the project is relevant for all young people with various long-term conditions. Young people with asthma, ADHD (Attention Deficit Hyperactivity Disorder), type 1 diabetes, epilepsy, congenital heart disease and young people who have undergone organ transplantation will be studied initially.
