Perhit - PERson-centredness in hypertension management using information technology
Short description
Randomized and controlled trial in primary care based on patient participation in hypertension care through daily self-monitoring of blood-pressure, symptoms, side-effects and life-style. An interactive web-based system for use in mobile phones is used to support self-management of hypertension. Results from a pilot study with 50 participants could show that the patients during follow-up consultations took on an active role, sharing to a large extent observations, reflections and questions. This means a different foundation for what the conversation may be all about– and for what subjects can be discussed. Further, after a eight weeks use of the system significant lower blood pressure could be shown. PERHIT tests use of the system in a larger group, about 900 patients are randomized to different groups.
The system consists of: (a) mobile phone (patient's own) for self-reporting of blood pressure, symptoms, medication adherence, side effects, lifestyle and well-being along with individualized reminders and encouraging messages for better lifestyles; (b) sphygmomanometer; (c) database for real-time recording of self-reported data entered by mobile phone (d) web-based feedback system that graphically displays collected self-reported data via the Internet accessible to patient and doctor/nurse.
