Short description

This project aims to identify knowledge crucial for the development of sustainable person-centered healthcare systems by combining research at the national (macro) level, the organizational (meso) level, and the individual (micro) level. Existing efforts to prevent involuntary loneliness and social isolation will be identified, and new models will be developed and tested. A central aspect in the project is the inclusion of elderly citizens in co-creating welfare services that prevent involuntary loneliness and social isolation. We aim to deepen these insights, not only to understand how person-centered care can be delivered in challenging situations but also to explore how we can develop sustainable systems for person-centered care that can function under other circumstances, such as the reorganization of primary healthcare services.
